APPLICATION FORM FOR PROTECTION OF PERSONAL DATA

With regards to your below requests within the scope of your rights under Article 11 of the Law No. 6698 on Protection of Personal Data (“Law”/”LPPD”), you can submit an application to us via the methods and procedures explained herein.

As the relevant person, you have the following rights under Article 11 of the Law No. 6698 on Protection of Personal Data:

1. To learn whether or not your personal data are being processed,
2. To request information on the procedure, if your personal data are being processed,
3. To obtain information on the purpose of processing of personal data and find out whether personal data is being used in line with the purpose of their collection,
4. To obtain information about third persons to whom personal data are being transferred domestically or abroad,
5. To request the correction of personal data that may be incompletely or inaccurately processed,
6. To request the deletion or destruction of personal data within the scope of the provisions set forth in the Article 7 of the Law,
7. To request notification of the procedures set out under sub-paragraphs “d” and “e” of Article 11 of the Law to third parties to whom the personal data has been transferred,
8. To object to the occurrence of a result which is detrimental to the relevant person as a result of analysing of the processed data exclusively through automated systems,
9. To request indemnification for damages in the case that damages are sustained as a result of unlawful processing of personal data.

You may exercise your rights via the following methods and procedures.

In the event that you apply to us in accordance with the procedures and principles stated in this form; your request will be dealt with and concluded free of charge as soon as possible and no later than in thirty days, depending on the nature of your request. However, in case the process requires any additional cost, the fee specified in the tariff issued by the Board of Protection of Personal Data may be requested.

This application form was issued to determine your relationship with our Company, to determine your personal data as the relevant person, to reply to your requests in the application, to evaluate and finalize your application and to reply to your application accurately and within the legal time-frame. Our Company reserves its right to request additional information and documents within the scope of the application for the purpose of determining the identity and authorization and for the evaluation of the application. The documents and information regarding your requests submitted within the scope of your application must be submitted by an authorized person and must be accurate and up-to-date. Our Company shall have no liability in case of an authorized application or if the information are detected to be not accurate and up-to-date, in which cases our Company’s right to reject the application is reserved.  The information and documents stated in this form and submitted to us will be processed by our Company in a restrictive manner only for the purposes of evaluating, replying to and finalizing the application you made pursuant to article 13 of the Law.  Information obtained via this form and the application may be collected in writing, verbally, or via an electronic or physical medium. In accordance with the examination conducted in this regard, the relevant information may be shared with the company and its affiliates, as well as third person service providers and companies such as law firms, in order to finalize the application.
